anyone know how to fix a phone or something like that?

i have a Samsung SGH-T629 under a T-Mobile contract and for no reason whatsoever, the pin number is blocked and i cant acces the phone at all, it turns on and everything but it wont go any further saying pin is blocked, then it says to put in a PUK number, i never changed it so obviously it is default (00000000) but it says the PUK number is incorrect. so i cant access or change the PIN number without getting thew PUK number correct, and i never changed it but it wont work, any help guys?

it was an RMA phone after my original broke, and i got it fixed so thanks for helping :D
